Living with Diabetes
The Novo Nordisk Patient Support Program is designed to support patients through personalized interactions. Crafted to meet the patients individual needs, the outcome of this experience is triggered based on where the patient is in their treatment journey. The objective: improve stay time on NNI medications, support patients as they transition (dose-up/add/switch medications) and/or build patient confidence in their treatment plan.
Above is a simplified map representing the user experience (UX) of the program. The patient enters the experience through an email sent to the targeted audience. Through that email the patient will enter tool 1, where they explore how they’re doing on their current treatment plan and consider anything they want to do differently and/or discuss with their health care provider. Once tool 1 is completed, the patient will download a customized doctor discussion guide and receive email 2, summarizing the information obtained in tool 1. Based on the input received in tool 1, the patient may then be opted to continue to the tool 2 experience, a branded version of tool 1 where the patient selects the specific medication they are currently taking. The patient can then opt in to receive email 3, containing medication recommendations to fit their individual needs.
Tool 1 allows the patient to explore how they’re doing on their current treatment plan, and consider anything they want to do differently and/or discuss with their health care provider.
The graphic above and below represents the dynamic elements of the output page found at the end of tool 1. The recommended resources and discussion points vary for each patient.
Based on the input received in tool 1, the patient may then be opted to continue to the tool 2 experience, a branded version of tool 1 where the patient selects the specific medication they are currently taking. The patient can then opt in to receive email 3, containing medication recommendations to fit their individual needs. Recommendations are based on whether the patient needs to increase their current dose, add a new medication to their treatment plan, or switch their medication entirely. The patient then brings their findings to their health care professional to discuss the direction of treatment.
